Se acepta Medicare, Medicaid, Medicaid-MCO, y algunos seguros privados para todos los servicios que se consideren médicamente necesarios. Se requieren copagos en el momento del servicio. Es posible que haya tarifas con descuento disponibles para los pacientes que pagan por sí mismos en una escala con tarifa de escala móvil basada en los ingresos del hogar. Los servicios en PEP se proporcionan sin costo a las personas sin seguro.

How it Works
What is a Sliding Scale?
A sliding scale adjusts the fee you pay for services based on your income. This ensures that our services remain affordable for all clients, especially those with lower income levels.
Acceptable Forms of Proof of Income (POI)
To qualify for our sliding scale fees, you will need to provide proof of your income. Here are the acceptable forms of documentation:

Tax Return: Most recent year (only if it was professionally prepared by a licensed tax preparer or if you have the electronic tax software submission page).

W-2: Most recent year.

Paystubs: 2 bi-weekly or 4 weekly.

SSI / SSDI Letter or 1 Bank Statement Showing Deposit.

IDES (Illinois Department of Employment Security): Unemployment Letter.

Pension / Retirement Letter.

Print out of Social Security Earnings: from the SSA website Social Security Administration, must be within the last year.

HUD Approved Housing Agreement: Showing income or amount of assistance granted.
Case-by-Case Acceptable POI
In certain situations, we may accept additional forms of proof of income on a case-by-case basis:

Alimony Letter: If you do not work.

Child Support Letter: If you do not work.

Signed Letter from Employer on a Letterhead: If paid in cash.

Letter from Public Aid for Food Stamps.
Determining Family Size
Family size impacts your eligibility and the fee scale. It is determined by how many dependents you claim on your most recent Tax Return. If providing other POI, you can only claim 1 additional dependent (i.e., 2 people total including yourself).
